Name Change Instructions

1) Create a new Name Change Request by clicking the "APPLY NOW" button located below.

2) Fill out the form and answer all questions to the best of your ability.

3) When you're done, click "Submit the topic" at the bottom!

Approved name changes will be notified within 48 hours of submission.

You may only apply for a name change ONCE per-calendar month.
